Mrs. Teresa Hazen, 48, of Spring Lake passed away Thursday, February 26, 2015 at her home. She was born in Star, Idaho and attended Ontario High School. Teresa worked at the K-Mart in Ontario, OR until moving to Spring Lake in 2010. She was a member of Gourd Springs Baptist Church in Spring Lake. Teresa was preceded in death by her daughter, Ashley Gentry.

Memorial service will be 3:00 PM Sunday, March 1, 2015 at Gourd Springs Baptist Church in Spring Lake with Pastor Ronnie Stewart officiating. The family will receive friends after the service at the church and other times at the home.

Teresa is survived by her husband, Randy Hazen of the home, son, Jayden Gentry of the home, daughter, Heather Clark and husband Bobby of Weiser, Idaho, 2 grandchildren, Dwayne Gentry, Clarice Clark, step-sons, Justin and Cody Hazen both of Warner Robins, GA, mother, Illena Hazen of Eugene, OR, sisters, Susan Hicks of Eugene, OR and Lisa Panayotoff of LaGrange, OR, brother, Jeff Smith of Emmett, Idaho.

In lieu of flowers, memorials may be made to Gourd Springs Baptist Church, 4575 Ray Rd., Spring Lake, NC 28390.
