Samuel Ray Velazquez, 5 months, of Willow Spring passed away Saturday, August 23, 2014 at Wake Medical Center in Raleigh. He was born March 17, 2014 in Raleigh, the son of Angela Velazquez and Ediberto Velazquez.

In addition to his parents, Samuel is survived by his sister, Isabelle Hope Velazquez of the home, maternal grandmother, Deborah O'Neil of Fayetteville, maternal great-grandmother, Betty Daugherty of Fayetteville, paternal great-grandmother, Luz Maria Santiago of Puerto Rico, and loving aunts, uncles and cousins.

A memorial service will be held at 6:00 PM Wednesday, August 27, 2014 at O'Quinn-Peebles-Phillips Chapel in Lillington. The family will receive friends after the service.
